It may be time to obtain some help, our friendly staff will be able to work with you at your own pace, ensuring that your dignity and safety is maintained through the process. A person centred care plan will be discussed with you and those involved in your care.

MEDICATION
Our friendly staff can assist in ensuring that medication is taken appropriately as prescribed.
Our friendy staff can remind you when to take the medication, read the small print on labels, open difficult medication containers and record what medications have been taken.
Our friendly staff can monitor you for any immediate reaction or symptoms after taking your medication and provide neccessary support.

FOOD PREPARATION
Maintaning your diet and nutrition can be challenging for you. Our friendly staff can assist in planning and preparing meals to meet general or specific nutritional and dietary needs.

COMPANIONSHIP
Having a bit of company can improve your social life and well being. Our friendly staff can do fun things with you either in the community or in your own home, such as solving puzzles, playing board games, watching movies, researching family history, chatting about current events, reading together ,light gardening, gentle walk..etc

SHOPPING
Our friendly staff can help by accompanying you during shopping trips .
If you would rather stay at home, our friendly staff will work with you to do your shopping list and do the shopping for you

HOSPITAL DISCHARGE
Properly managing the transition home following discharge from the hospital or rehabilitation facility is essential for a safe and effective recovery. Our team will work with the hospital discharge planners and social workers to develop a Care Plan to ensure your loved one has a successful recovery. Having a robust support plan in place for transitioning home is an effective way to manage post-hospital recovery risk.

PALLIATIVE CARE
Through our Palliative Care Services, we will work with you, your loved ones and your GP to plan your holistic care. Our friendly staff will be available 24/7 to provide the required tender loving care.
